Job Summary:
The Berkshire Healthcare NHS Foundation Trust is seeking a highly skilled and compassionate Senior Mental Health Practitioner to join our CAMHS Rapid Response team in Slough. As a key member of our team, you will be responsible for providing high-quality mental health assessments and short-term interventions to young people experiencing mental health crises.
Key Responsibilities:
- Conduct comprehensive psychosocial crisis assessments for young people who have presented in crisis.
- Offer short-term intervention to stabilize the crisis, including follow-up appointments and multi-agency consultation-liaison.
- Identify ongoing care plan needs for young people and their families, ensuring they are referred to appropriate services.
- Maintain and develop key relationships with professionals external to the area of service, as well as patients and their families.
- Be responsible for the accurate maintenance of clinical record keeping and ensuring all systems, records, and data are stored and transferred correctly.
Requirements:
- A degree in a relevant field, such as Social Work, Nursing, or Occupational Therapy, as well as a core profession.
- Experience in working with mental health and complex needs.
- Demonstrable skills in mental health assessments and formulation.
- A full UK driving license with access to a vehicle.
