About the Role:
We are seeking a highly motivated and experienced Community Outreach Specialist to join our team at Change Grow Live. As a key member of our outreach team, you will play a vital role in engaging with individuals who have adopted 'street-based living' and supporting them in accessing appropriate services.
Key Responsibilities:
- To develop and maintain relationships with service users, understanding their needs and providing tailored support to help them achieve their goals.
- To work collaboratively with internal and external stakeholders, including other agencies and professionals, to ensure a coordinated approach to service delivery.
- To identify and address the complex needs of service users, including substance misuse and mental health issues.
- To provide information, guidance, and support to service users, empowering them to make informed decisions about their lives.
- To work in partnership with service users to develop and implement personalized plans, promoting sustainable change and positive outcomes.
About You:
We are looking for an individual with a strong understanding of substance misuse issues and experience of working within a similar field. You will have excellent communication and interpersonal skills, with the ability to work with confidence and patience with clients who may be unwilling to engage with services.
Requirements:
