Key Responsibilities:
- Collaborate with individuals, their families, providers, and other healthcare professionals to develop and implement personalized care plans.
- Use your excellent relationship management skills to build trust and rapport with service users, their families, and other stakeholders.
- Work in a strengths-based way, focusing on empowering individuals to take control of their lives and make informed decisions about their care.
- Conduct thorough assessments and risk assessments to identify the needs of service users and develop effective interventions.
- Develop and maintain strong relationships with community resources, such as libraries and other local facilities, to support service users in their daily lives.
Requirements:
- Qualified Social Worker with a strong understanding of mental health principles and practices.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and other stakeholders.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions.
- Current driver's license and access to a reliable vehicle, with the ability to travel to various locations within Hampshire and the Isle of Wight.
