Job Summary

We are seeking a qualified Social Worker to join our Mental Health Team in Havant. As a key member of our team, you will work collaboratively with individuals, families, and healthcare professionals to promote independent and safe living within the community.

Key Responsibilities
  • Provide person-centred and strengths-based social work services to individuals and families.
  • Develop and implement effective care plans to meet the needs of service users.
  • Work collaboratively with healthcare professionals and other agencies to ensure seamless service delivery.
  • Conduct assessments and provide advice and guidance to service users and their families.
  • Participate in multi-disciplinary teams to develop and implement service plans.
Requirements
  • Qualified Social Worker with a degree in Social Work or equivalent.
  • Experience of working in a mental health setting or with vulnerable adults.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
  • Strong assessment and planning skills.
