Job Summary

We are seeking a highly skilled and experienced Clinical Team Leader to join our Frailty Service at Kings College Hospital. The successful candidate will be responsible for leading a team of healthcare professionals in delivering high-quality patient care and developing clinical protocols to enhance the continuity and standard of specialist care.

Key Responsibilities
  • Leadership and Management
    • Lead a team of Frailty Practitioners in the delivery of the King's Frailty service, providing day-to-day line management and support.
    • Develop policies and protocols to improve patient care.
    • Liaise effectively with all other disciplines within the Trust and at a local level.
  • Clinical Practice
    • Provide expert advice and support to older people and people with frailty, their families and other healthcare professionals applying comprehensive assessment of the older person and frailty management.
    • Manage a patient caseload.
    • Make and receive referrals.
    • Assess and facilitate timely discharge for patients.
    • Prevent unnecessary admissions to an acute bed by providing appropriate support and direction for healthcare professionals, patients and their families across the frailty pathway.
  • Education and Audit
    • Lead and coordinate the development, implementation and evaluation of care pathways and systems of interdisciplinary documentation.
    • Provide information for patients, carers and healthcare professionals about all aspects of the frailty service.
    • Ensure appropriate information and educational literature is available to help patients make informed choices as to their treatment plan and continuing management.
Requirements
  • Professional registration – Nurse or Physiotherapist or Occupational Therapist.
  • Minimum undergraduate degree relevant to profession.
  • Post-graduate degree relevant to specialist area, or willing to work towards.
  • Registered non-medical prescribing qualification.
  • Experience in managing a clinical team.
  • Relevant, demonstrable experience in specialty area.
  • Ability to build up a rapport with patients and the inter-professional team.
  • Patient centred approach.
  • Excellent communication skills both oral and written.
