About the Role
NHS Professionals Limited is seeking a skilled Community Health Nurse to join our team. As a Community Health Nurse, you will be responsible for providing high-quality patient care in a community setting.
Key Responsibilities
- Assess and monitor patient conditions, arranging diagnostic tests as needed.
- Recognize and respond to patient deterioration, taking appropriate action to escalate care.
- Manage emergency situations, demonstrating excellent analytical skills and sound decision-making.
- Develop and implement individualized care plans, promoting patient autonomy and well-being.
- Collaborate with multidisciplinary teams to ensure seamless patient care.
Requirements
- A valid UK driving license and access to a car.
- A minimum of 6 months paid experience in a community environment within the last 2 years.
- Current NMC registration and adherence to professional standards.
