About the Role

We are seeking a highly skilled and dedicated Community Health Nurse to join our team at the Northern Care Alliance NHS Foundation Trust. As a key member of our community nursing team, you will play a vital role in delivering high-quality care to patients in their own homes and communities.

Key Responsibilities
  • Assess and manage the health needs of patients with complex nursing requirements, including those with long-term conditions and end-of-life care.
  • Work autonomously to plan and deliver care, using your clinical skills and expertise to make informed decisions.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.
  • Provide education and support to patients, families, and carers to promote self-care and independence.
  • Contribute to the development and implementation of community care pathways and protocols.
Requirements
  • Registered Nurse or Community Specialist Practitioner degree.
  • Up-to-date professional portfolio demonstrating evidence of reflective practice.
  • Ability to travel to different geographical locations across the Trust footprint.
  • Excellent communication and interpersonal skills.
  • Leadership potential and ability to work autonomously.
