We are seeking an experienced Care Coordinator to join our dedicated team in Albrighton, Wolverhampton. As a skilled Care Coordinator, you will be responsible for managing a caseload of care packages and understanding the varied nature and fast-pace of the role.
Key Responsibilities- Manage rota's for care teams and respond efficiently to any day-to-day changes
- Act as the first point of contact for service users, their families, and other healthcare professionals
- Apply communication and relationship building skills to effectively manage care delivery
- Work well under pressure, using initiative and thinking on your feet
- Experience in care coordination, preferably in an office-based setting
- Ability to travel across the West Midlands region, with a valid driving licence and access to a vehicle
- Commitment to supporting the out of hours on call service, on a 1 in 7 rota basis
- Competitive salary between £28,290 - £33,290 pa, including payment for on-call
- Bonus scheme of up to £2,000 pa
- 25 Days Annual Leave, plus Bank Holidays
