Position Overview

We are seeking a dedicated and proactive Integrated Care Community Coordinator to join our dynamic team. The ideal candidate will possess a strong commitment to addressing health disparities and enhancing health equity. This position offers a unique opportunity to be part of a collaborative team, providing comprehensive administrative support to facilitate the effective operation of our Integrated Care Community. The responsibilities will be diverse, requiring adaptability and a willingness to engage in various tasks. Key duties will include: assisting the core leadership team; providing telephone support, including direct patient interactions; organizing meetings and preparing necessary documentation; creating presentations and visual materials; and managing administrative tasks during meetings, including multi-disciplinary team gatherings, record maintenance, and actively contributing to our population health initiatives.

Key Responsibilities

- Provide clerical assistance to the Integrated Care Community Core Team to ensure seamless operations.

- Deliver non-clinical telephone support to patients, allowing clinicians to concentrate on their clinical responsibilities while ensuring a patient-centered service.

- Offer administrative support to the Integrated Care Community and team members for various initiatives, including data management, presentation preparation, and documentation.

- Prepare agendas, take minutes, and compile meeting documentation for a variety of gatherings, including Integrated Care Community Team Meetings and Multi-Disciplinary Team Meetings.

- Maintain an organized archive to facilitate document retrieval in accordance with organizational policies and procedures, utilizing both electronic and manual systems as necessary.

- Engage regularly with a diverse range of partners across our Integrated Care Community.

Most importantly, you will be part of a vibrant and supportive team, where your ideas for improvement are valued, and you will contribute to the ongoing development of our Integrated Care Community in collaboration with our stakeholders.
