About This Role
We are seeking a dedicated and experienced Health Visitor to join our team in South Crewe and Nantwich. In this vital role, you will play a key part in promoting the health and well-being of children and families within our community.
Key Responsibilities:
- Provide comprehensive health assessments and guidance to families throughout their child's development.
- Develop and implement individualized care plans, addressing the unique needs of each child and family.
- Collaborate with a multidisciplinary team of healthcare professionals to ensure holistic care for children and families.
- Educate families on various aspects of child health, including nutrition, safety, and development.
- Advocate for the well-being of children and families, ensuring their voices are heard and their needs are met.
Qualifications & Experience:
- Registered Nurse (Adult or Child/RSCN) or Registered Midwife.
- Bachelor of Science in Public Health Nursing - Health Visiting or equivalent.
- Nurse Prescriber qualification (V100).
- Proven experience managing a Health Visiting caseload.
- Strong leadership, mentorship, and communication skills.

- Excellent interpersonal and communication skills.
- Ability to work effectively both independently and as part of a team.
- Passion for promoting the health and well-being of children and families.
