Job SummaryThe successful candidate will be responsible for providing expert medication management services to our patients, working closely with our clinicians and other healthcare professionals to ensure seamless care. You will be expected to undertake medication reviews, provide patient education, and implement prescribing decisions in line with current guidelines and best practices.
Main Responsibilities- Conduct structured medication reviews for patients with complex polypharmacy, implementing prescribing decisions and requesting appropriate tests as needed.
- Reconcile medicines following hospital discharge, working with patients and community pharmacists to ensure timely medication receipt.
- Support long-term condition reviews via face-to-face and telephone appointments, contributing to reductions in medicine-related hospital admissions and re-admissions.
- Identify and address medicine-related issues, assisting in managing patient and healthcare professional queries.
- Contribute to service improvement within enhanced services and Quality and Outcomes Framework (QOF) initiatives.
We are looking for a highly skilled and motivated Clinical Pharmacist with the following qualifications and experience:
- Member of the Royal Pharmaceutical Society (RPS)
- UK-accredited MPharm degree or OSPAP qualification
- Clinical Pharmacy Diploma
- 52 weeks of pre-registration training in England, Scotland, or Wales, signed off as satisfactory by your tutor
- GPhC registration assessment with a pass result
- Ability to demonstrate practical experience of dealing with patients, evidence of Continuing Professional Development, and experience of working in General Practice
