Morrab Surgery is a dynamic and patient-focused healthcare provider dedicated to delivering high-quality care to our community. We are seeking an experienced and skilled Practice Nurse to join our team and contribute to the delivery of exceptional patient care.

Job Summary

The Practice Nurse will play a vital role in providing and maintaining a high standard of nursing care to our patients, with a focus on long-term condition management and acute problems. This includes screening, assessment, treatment, and education, as well as taking an active role in supporting the practice to maximize achievement against local and national targets.

Main Responsibilities

  • Provide routine nursing care to patients in accordance with clinical-based evidence, NICE, and NSF guidelines.
  • Diagnose and manage acute and chronic conditions as required.
  • Review patient medications to enhance compliance and provide guidance on the use of prescribed and over-the-counter medications.
  • Coordinate the provision of travel medicine services and request pathology services as needed.
  • Oversee the provision of chronic disease clinics and deliver patient care as necessary, referring patients to secondary or specialist care when required.
  • Maintain accurate clinical records in accordance with existing legislation.
  • Ensure the effective use of read codes and chronic disease registers.
  • Develop, implement, and embed health promotion and wellbeing programs.
  • Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
  • Support the team in dealing with clinical emergencies and recognize, assess, and refer patients presenting with mental health needs.
  • Implement vaccination programs for adults and children and support patients in the use of their prescribed medicines or over-the-counter medications.
  • Liaise with external services and agencies to ensure patients receive appropriate support.
  • Delegate clinical responsibilities appropriately, ensuring safe practice and tasks are within the scope of practice.
  • Support the clinical team with safeguarding matters, in accordance with local and national policies.
  • Understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately.
  • Deliver opportunistic health promotion where appropriate.
