Job Description:

We are seeking a skilled Mental Health Practitioner to join our Community Rehabilitation Team. As a Mental Health Practitioner, you will be responsible for providing rehab interventions to patients on the discharge pathway and those living in the community.

Responsibilities:

  • Provide rehab interventions to patients on the discharge pathway and those living in the community.
  • Work collaboratively with the Community Rehabilitation Team to achieve patient outcomes.
  • Develop and implement individualised care plans to meet the needs of patients.
  • Conduct regular assessments and reviews to ensure patient progress and adjust care plans as necessary.

Requirements:

  • Current registration with the Nursing and Midwifery Council (NMC) as a Mental Health Nurse.
  • Fully enhanced DBS.
  • Community experience.
