Key Responsibilities- Assess, plan, and deliver interventions in collaboration with the multidisciplinary team.
- Engage with service users and their carers to promote recovery and well-being.
- Work autonomously within defined boundaries, taking ownership of delegated tasks and responsibilities.
- Contribute to the development and implementation of care plans, ensuring a person-centered approach.
- Collaborate with external agencies to promote partnership working and holistic care.
- Foundation Degree in Health and Social Care or a related field.
- Experience working with service users with mental health needs, preferably in a multidisciplinary team setting.
- Excellent communication and problem-solving skills, with the ability to work independently and as part of a team.
- Understanding of mental health issues, risk assessment, and the Care Programme Approach.
