Key Responsibilities
  • Deliver personalized care and support to patients in their own homes, focusing on their physical, emotional, and social needs.
  • Monitor symptoms, administer medications, and maintain accurate records to ensure the best possible quality of life for our patients.
  • Work collaboratively with our Clinical Nurse Manager and other healthcare professionals to develop and implement individualized care plans.
  • Provide emotional support and guidance to patients and their families, promoting a sense of comfort and well-being.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in palliative care.
