Job Title: Apprentice Family Practitioner

We are seeking a highly skilled and motivated Apprentice Family Practitioner to join our team at The Phoenix Practice.

Key Responsibilities:

  • Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems.
  • Screening patients for disease risk factors and early signs of illness.
  • Developing care plans for health management in consultation with patients and in line with current practice disease management protocols.
  • Providing counselling and health education to patients.
  • Making appropriate and timely referrals of patients for further opinion and treatment.
  • Recording clear and contemporaneous consultation notes to agreed standards.
  • Appropriate medicine prescribing for both acute and repeat requirements in accordance with the practice prescribing formulary and guidelines.
  • Participating in training plans of GP registrars, medical and nursing students as required.

Requirements:

  • Strong communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Excellent problem-solving and decision-making skills.
  • Knowledge of current practice disease management protocols.
  • Ability to work with electronic systems.
