Job Title: Practice Nurse

We are seeking an experienced Practice Nurse to join our team at our GP Practice in Wales. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the delivery of our practice's mission to 'Health and wellbeing for all who live in our community.'

About the Role

This is an exciting opportunity for a skilled and motivated Practice Nurse to join our dynamic team. As a Practice Nurse, you will be responsible for delivering a range of clinical services, including chronic disease management, vaccinations, and health promotion. You will work closely with our GP team and other healthcare professionals to provide comprehensive care to our patients.

Key Responsibilities
  • Deliver high-quality patient care, including clinical examinations, assessments, and treatment plans.
  • Work collaboratively with the GP team and other healthcare professionals to provide comprehensive care to patients.
  • Contribute to the development and implementation of health promotion strategies and chronic disease management plans.
  • Provide education and support to patients, carers, and families to promote healthy lifestyles and self-care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Registered Practice Nurse with experience of working in Primary Care.
  • Knowledge of health promotion strategies and experience of chronic disease management, vaccinations, and immunisations.
  • Experience of working within a nursing team and contributing to the delivery of high-quality patient care.
