About the RoleThis is an exciting opportunity for a motivated and compassionate Practice Nurse to join our team. You will be responsible for providing nursing care to patients, conducting health assessments, and developing care plans to meet individual needs. You will also work closely with our healthcare team to ensure seamless care delivery and contribute to the development of our practice's policies and procedures.
Key Responsibilities- Deliver high-quality nursing care to patients, including health assessments, wound management, and chronic disease management.
- Develop and implement care plans to meet individual patient needs, in collaboration with our healthcare team.
- Conduct health education and promotion activities to support patients in managing their health and well-being.
- Work closely with our healthcare team to ensure seamless care delivery and contribute to the development of our practice's policies and procedures.
- Participate in quality improvement initiatives and contribute to the development of our practice's quality improvement plan.
To be successful in this role, you will need:
- Registration with the Nursing and Midwifery Council (NMC) as a Practice Nurse.
- Experience of working in a primary care setting, preferably in a GP practice.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, colleagues, and other healthcare professionals.
- Ability to work autonomously and as part of a team, with a flexible approach to work.
- Knowledge of health promotion strategies and experience of chronic disease management, vaccinations, and immunisations.
