About the Role

This is an exciting opportunity for a skilled and motivated Practice Nurse to join our dynamic team. As a Practice Nurse, you will be responsible for delivering a range of clinical services, including chronic disease management, vaccinations, and smear taking. You will work closely with our GP team and other healthcare professionals to provide comprehensive care to our patients.

Key Responsibilities
  • Deliver high-quality patient care, including clinical examinations, assessments, and treatments.
  • Manage and coordinate patient care, including referrals to other healthcare services as needed.
  • Contribute to the development and implementation of clinical policies and procedures.
  • Participate in quality improvement initiatives and contribute to the continuous improvement of patient care.
  • Work collaboratively with the GP team and other healthcare professionals to provide comprehensive care to patients.
Requirements
  • Registered Practice Nurse with experience of working in Primary Care.
  • Knowledge of health promotion strategies and experience of chronic disease management, vaccinations, and smear taking.
  • Experience of working within a nursing team.
