About the Role

We are seeking a highly skilled and motivated Clinical Advisor to join our Adult Community Nursing Service in Hounslow and Richmond areas. As a Clinical Advisor, you will play a crucial role in providing high-quality, skilled nursing care to housebound patients with complex needs, enabling them to remain in their own homes.

Key Responsibilities
  • Deliver high-quality nursing care to patients with complex needs, working closely with specialist nurses, allied health, and social care professionals to meet their needs without unnecessary attendance in A&E or hospital admission.
  • Collaborate with integrated rapid response teams, community matrons, and social service colleagues to ensure seamless care to our patient group.
  • Work independently as part of the Community Nursing Team, covering a particular geographical area, and provide first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care.
  • Support the Community Nursing Team Manager in the on-going planning, implementation, and evaluation of the holistic needs of patients, families, and carers.
Requirements
  • Registered General Nurse with a strong commitment to giving the highest standard of patient care.
  • Compassionate towards patients and their families, with excellent communication skills and the ability to work in a team.
  • Motivated towards personal development, with a full UK driving license and access to a car.
