Key Responsibilities- Provide comprehensive nursing care to patients in their own homes, including assessments, treatments, and ongoing support.
- Work collaboratively with other healthcare professionals to develop and implement individualized care plans.
- Conduct regular reviews and assessments to ensure patients are receiving the best possible care.
- Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination.
- Participate in ongoing education and training to stay up-to-date with the latest clinical practices and guidelines.
- Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC)
- Experience of recent community/acute healthcare gained through education or work experience placement or previous employment
- Awareness of professional issues and recent developments in the NHS
- Knowledge of up-to-date clinical practice
- Sound communication and interpersonal skills (verbal and written)
- Ability to cope with change
- Ability to use own initiative
- Able to drive with full Driving Licence
- Ability to motivate and organise others to ensure best to work under pressure
