We are seeking a highly skilled and dedicated Community Nurse to join our team at Kent Community Health NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality patient care and supporting the health and wellbeing of individuals in their own homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their own homes, including assessments, treatments, and ongoing support.
  • Work collaboratively with other healthcare professionals to develop and implement individualized care plans.
  • Conduct regular reviews and assessments to ensure patients are receiving the best possible care.
  • Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date with the latest clinical practices and guidelines.
Requirements
  • Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC)
  • Experience of recent community/acute healthcare gained through education or work experience placement or previous employment
  • Awareness of professional issues and recent developments in the NHS
  • Knowledge of up-to-date clinical practice
  • Sound communication and interpersonal skills (verbal and written)
  • Ability to cope with change
  • Ability to use own initiative
  • Able to drive with full Driving Licence
  • Ability to motivate and organise others to ensure best to work under pressure
  • Flexible and adaptable
