About the Role

We are seeking an experienced Mental Health Practitioner to join our team in Kent and Medway NHS and Social Care Partnership Trust. As a Community Mental Health Practitioner, you will work in Primary Care Networks (PCNs) providing mental health support to patients in a community setting.

With a strong background in mental health, you will be responsible for delivering high-quality interventions and working collaboratively with multidisciplinary teams to provide excellent care to our service users.

Key Responsibilities
  • Deliver evidence-based practice in caring for patients with mental ill health in a community setting
  • Communicate complex and sensitive information effectively to patients, carers/families, and multidisciplinary teams
  • Work collaboratively with PCNs to provide mental health support and interventions
