Job Description

Improving Health LTD is seeking a skilled and compassionate Health and Wellbeing Coach to join our team in delivering proactive, personalized, and integrated health and social care services to our local population.

Key Responsibilities
  • Support local residents in managing their health conditions, including diabetes, through personalized coaching and guidance on diet, exercise, mobility, sleep hygiene, weight management, and mental wellbeing.
  • Provide one-to-one and group health coaching support to individuals, empowering them to take control of their health and wellbeing.
  • Work closely with the social prescribing team to ensure individuals have access to support addressing wider social needs that impact their health and wellbeing.
  • Promote a good understanding of health coaching across practices, the PCN, and wider system, acting as an advocate for this role and its impact.
  • Develop strong links with local statutory, voluntary, and community organizations to enable referred patients to access the support they need.
  • Seek regular feedback on the quality of service and impact of health coaching, encouraging individuals, families, and carers to provide feedback and share their stories.
  • Identify opportunities for service development and innovation, working with leads to continually improve how the service supports the local population.
Requirements
  • Coaching/counselling qualification or other relevant qualification and experience of coaching.
  • Experience of using coaching approaches/frameworks and models or other helping strategies.
  • Experience of working in health and social care settings or similar.
  • Experience of coaching and motivating individuals to achieve their individual health, care, or lifestyle goals.
  • Ability to actively listen, empathize with people, and provide person-centered support in a non-judgmental way.
  • Ability to communicate effectively, both verbally and in writing, with people, their families, carers, community groups, partner agencies, and stakeholders.
  • Ability to identify risk and assess/manage risk when working with individuals.
  • Able to provide a culturally sensitive service, supporting people from all backgrounds and communities, respecting lifestyles and diversity.
  • Able to work with others to find creative solutions to issues.
  • Demonstrable commitment to professional and personal development.
  • Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on communities.
  • Understanding of, and commitment to, equality, diversity, and inclusion.
  • Knowledge of how the NHS works, including primary care.
Working Arrangements

The post holder will work in a hybrid remote setting in London, SE5 0HF, and will be required to travel across sites as needed.
