About the Role

We are seeking an experienced mental health clinician to join our team as a Primary Care Mental Health Practitioner in Gateshead Inner West PCN.

This is an exciting opportunity to work in a primary care setting, providing integrated services to our local population and delivering more personalised, coordinated health and social care.

Main Responsibilities
  • Assess and provide brief interventions for working-age adults and older patients
  • Work across the 5 GP surgeries in the PCN, developing relationships with local services and partner agencies
  • Offer navigation to local services and support the community mental health transformation model
  • Provide clinical supervision to clinical staff and contribute to the development of care pathways
Requirements
  • Degree or equivalent qualification in evidence-based psychological therapies
  • Post Graduate Degree or equivalent qualification
  • ENB 998/P126/127 Student Nurse Assessor or equivalent
  • Non-Medical Prescriber
  • Knowledge and understanding of equal opportunities legislation and policies
