Healthcare Support Worker

We are seeking a compassionate and caring individual to join our Reablement service as a Care Assistant. As a Care Assistant, you will play a vital role in supporting individuals to manage their personal care within their own homes, promoting independence and confidence.
Key Responsibilities
Supporting individuals who have been discharged from hospital or have experienced a change in circumstances, such as an illness or crisis, to regain their independence and confidence.
Working closely with individuals to identify their goals and develop a personalized care plan to achieve their desired outcomes.
Providing emotional support and companionship to individuals, promoting a sense of well-being and reducing feelings of loneliness.
Assisting individuals with daily living tasks, such as personal care, meal preparation, and household chores.
What You'll Need
No prior social care experience is necessary, as we are more interested in your compassion and ability to care. You will need to be resilient and willing to learn new skills and develop your knowledge as part of a close-knit team.
