About the Role

We are seeking a highly motivated and experienced Community Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with patients, families, and healthcare professionals to provide holistic and person-centered care.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care, developing individualized care plans that meet the unique needs of each patient.
  • Conduct thorough holistic assessments, identifying patients' physical, emotional, and social needs, and developing care plans that address these needs.
  • Work collaboratively with patients, families, and healthcare professionals to provide seamless and coordinated care.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals, ensuring effective communication and collaboration.
  • Participate in multidisciplinary team meetings and contribute to the development of care plans and strategies.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices, applying this knowledge to improve patient outcomes.
  • Contribute to the development and implementation of quality improvement initiatives, ensuring that patient care is safe, effective, and efficient.
Requirements
  • Registered Nurse with current NMC registration.
  • Experience working in a community setting, preferably in palliative care or wound care.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
  • Excellent assessment and problem-solving skills, with the ability to think critically and make sound judgments.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
