Key Responsibilities- Provide assessments, care, and treatment for individuals with learning disabilities, mental health issues, and challenging behaviors in a community setting.
- Work closely with multidisciplinary teams to develop and implement care plans that promote independent living and recovery.
- Support individuals with communication difficulties or non-verbal communication needs.
- Collaborate with healthcare professionals, carers, and other stakeholders to ensure seamless care delivery.
- Contribute to the development of local health services to promote inclusive care for individuals with learning disabilities.
- NVQ Level 3 qualification or equivalent in health or social care.
- Experience of working with individuals with learning disabilities, mental health issues, and challenging behaviors.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team and adapt to changing situations.
- Competence in the use of IT software, including Microsoft Office and electronic patient records.
