We are seeking a highly skilled and motivated Community Nurse Specialist to join our Out of Hours team at Torbay and South Devon NHS Foundation Trust. As a key member of our team, you will be responsible for providing high-quality nursing care to patients in their own homes, enabling them to maintain their independence and quality of life.
Key Responsibilities- Assessment and Care Planning: Conduct holistic nursing assessments to identify patients' needs and develop personalized care plans to ensure safe and effective delivery of care.
- Co-ordination and Provision of Nursing Services: Ensure the effective co-ordination and provision of nursing services in the absence of the ISU Nurse Lead, including the planning and discharge of complex patients from secondary to primary care.
- Nursing Interventions and Liaison: Provide nursing interventions, assessment, liaison, referral, co-ordination, delegation of care, or placement in order to prevent hospital admissions and promote health education and well-being.
- Team Management: Manage a team of Community Nurses and SNR's in line with the trust policy, promoting a supportive and collaborative team environment.
The Out of Hours Nursing Team at Torbay and South Devon NHS Foundation Trust is a dynamic and supportive team that is passionate about delivering quality care to housebound patients. Requirements- Professional Qualifications: Hold a relevant nursing qualification and be registered with the NMC. Possess a Non-Medical Prescriber Qualification and Community Specialist Qualification.
- Skills and Experience: Demonstrate excellent communication and interpersonal skills, with the ability to work autonomously and prioritize workload without direct supervision. Possess a commitment to delivering high-quality care through an integrated and holistic approach.
- Personal Qualities: Be a dynamic, enthusiastic, and motivated individual who is passionate about delivering quality care to patients in their own homes.
