Key Responsibilities- Provide direct support to individuals in achieving their recovery goals
- Work collaboratively with the Multi-Disciplinary Team to deliver person-centred care
- Develop and implement individualised care plans
- Monitor and evaluate the effectiveness of care interventions
- Contribute to the development of the service and participate in quality improvement initiatives
