The Humber Teaching NHS Foundation Trust is seeking a highly skilled and experienced Nursing Specialist to join their Memory Assessment Service. The successful candidate will be responsible for providing high-quality assessment and treatment to individuals living with dementia in Hull and East Riding. The role will involve triage of referrals, assessment, formulation, delivery of diagnosis, provision of treatment interventions, forward care planning, and responding to the changing needs of patients and their carers. The successful candidate will also be responsible for forming good working relationships with partner services and the larger Older Peoples Mental Health Service. Delivering education to promote the wellbeing of those living with dementia and their supporters is also a key aspect of the role. This is a supervisory role with responsibility for the clinical supervision of team members. The ideal candidate will have specialist nursing knowledge of working with people with dementia, knowledge of relevant policies, guidelines, and drivers, and a willingness to work remotely and efficiently. The service is busy, and time management is vital to the implementation of the pathway. The successful candidate will have the opportunity to develop leadership skills by providing clinical supervision and appraisal to team members and ensuring local and national policies are implemented. The role requires the ability to recognize the signs and symptoms of physical and mental health issues that may affect cognition. The successful candidate will be expected to have a current NMC registration and a dementia-related qualification or be currently studying towards one. Experience of delivering diagnosis of dementia, undertaking cognitive assessment, and providing clinical supervision is essential. The ideal candidate will also have experience of working with 3rd sector colleagues and be able to work efficiently in a busy environment.
