As a key member of our team, you will provide specialist nursing care, support, and coordination of care to children and young people with complex healthcare needs and disabilities.
About the RoleAs a Community Children's Nurse, you will work collaboratively with children, young people, families, and the wider multi-disciplinary team to ensure care is provided as close to home as possible. You will provide high-quality, holistic, family-centered care to children and young people with a range of needs, including those with acute and short-term conditions, long-term conditions, disabilities, complex health conditions, and life-limiting and life-threatening illnesses.
Main Responsibilities- Manage a caseload of children and young people with complex healthcare needs and disabilities.
- Provide specialized, high-quality skilled nursing care, considering the physical, psychological, spiritual, and social needs of the child.
- Develop, implement, and evaluate individualized programs of care.
- Support the management of integrated acute and community care pathways for children with short-term conditions.
- Provide supervision and monitoring of junior staff and students.
- Work with families to provide emotional support and specialist clinical care, sensitive to the changing and ongoing needs of the child, siblings, parents, and other carers.
To be successful in this role, you will need:
- BSc Nursing or RN in Child or Learning Disability Nursing and current registration with the NMC.
- Experience in assessing and developing patient care plans.
- Experience in developing and delivering care and clinical interventions to patients.
- Experience of MDT working.
- Experience of teaching, training, and supervision/assessing junior staff and students.
- Evidence of recent CPD or desire to progress.
