Community Children's Nurse

We are seeking a skilled and enthusiastic Community Children's Nurse to join our integrated Community Children's Nursing (iCCN) team at Hampshire and Isle of Wight Healthcare NHS Foundation Trust.
The successful candidate will provide high-quality and holistic, family-centred care to children with nursing needs, including those with acute and short-term conditions, long-term conditions, disabilities, and life-limiting and life-threatening illnesses.
Main Responsibilities- Provide a high-quality in-reach and outreach nursing service to enable early, safe planned discharge from hospital and prevent unnecessary admissions.
- Support the management of integrated acute and community care pathways for children with short-term conditions.
- Supervise and monitor junior staff/students with support from the senior team.
- Work with families to provide emotional support and specialist clinical care, sensitive to the changing and ongoing needs of the child, siblings, parents, and other carers.
- Teach families/carers to carry out specific complex nursing care and procedures.

Person Specification- BSc Nursing or RN in Child or Learning Disability Nursing and currently registered with NMC.
- Experience in assessing and developing patient care plans.
- Experience in developing and delivering care and clinical interventions to patients.
- Experience of teaching, training, and supervision/assessing junior staff and students.
- Clinically competent at Band 5 level.
- Teaching, supervision, and assessing skills.
- Understanding of health needs of people in their care and health promotion.
