About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team at Farnham Park Health Group. As a key member of our primary care team, you will be responsible for delivering high-quality patient care and working collaboratively with our GPs and other healthcare professionals.

Key Responsibilities
  • Support GPs in delivering patient care using advanced autonomous clinical skills
  • Assess, diagnose, and treat patients presenting with acute illnesses
  • Manage and care for patients with long-term conditions
  • Prescribe and review medication in accordance with evidence-based practice and national guidelines
  • Provide information and advice on medication and treatment options
Requirements
  • Registered General Nurse with the NMC
  • Minimum 5 years post-registration experience
  • Recent experience in primary care, community, or emergency settings
  • Ability to work autonomously and as part of a team
