Job Title: Registered Nurse - Children's Community Services

We are seeking a qualified and experienced Registered Nurse to join our Children's Community Services team at University Hospitals of Derby and Burton NHS Foundation Trust.

Job Summary:

The successful candidate will be responsible for providing high-quality, patient-centered care to children and young people with complex health needs in the community setting. This will involve working closely with families, carers, and other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities:

  • Assess and manage the health needs of children and young people with complex conditions
  • Develop and implement individualized care plans in collaboration with families and other healthcare professionals
  • Provide education and support to families and carers on managing complex health conditions
  • Work closely with other healthcare professionals to ensure seamless care transitions
  • Participate in multidisciplinary team meetings to discuss patient care and develop care plans

Requirements:

  • Registered Nurse qualification (Band 5)
  • Experience working with children and young people with complex health needs
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work a variety of shifts, including nights, weekends, and bank holidays
