Your role will involve comprehensive nursing evaluations, high-quality care, guidance, and information provision to patients and their families, with the objective of supporting patients in their residences, thereby promoting maximum independence and optimal quality of life.
You will ensure the efficient coordination and delivery of nursing services in the absence of the Zone Nurse Lead and actively engage in the planning and transition of complex patients from secondary to primary care.
Collaboration with the Intermediate Care and community nursing teams will be essential to provide urgent nursing interventions, assessments, referrals, coordination, and delegation of care to avert hospital admissions.
Key Responsibilities
- Deliver high-quality care to patients in their homes while collaborating with a multidisciplinary team and partnering with primary and secondary care providers.
- Offer specialized clinical guidance to the multidisciplinary team.
- Conduct initial and ongoing assessments of the nursing needs of patients and their families in the community, considering physical, social, emotional, and cultural factors when devising care plans.
- Function as an independent practitioner in the design and execution of nursing treatment plans.
- Educate and support patients and caregivers on health and wellness topics, as well as self-care techniques.
- Perform complex nursing assessments, including continuing healthcare evaluations, medication reviews, prescribing, and palliative care.
- Utilize a diverse range of specialized nursing skills and knowledge, aligning practice with the organization's Medicines Policy for Registered professionals and the NMC Standards for Medicines management, ensuring that assessment and competence are maintained and documented as a nurse prescriber.
- Assist the Zone Lead with staff supervision, performance evaluations, and management of absences.
