About the Role

We are seeking an experienced and skilled Team Leader to join our team at Via Community Ltd. As a Team Leader, you will play a crucial role in supporting a team of committed practitioners who provide case management services to vulnerable individuals.

Key Responsibilities:

  • Lead a team of 5 outreach practitioners, providing guidance and support to ensure excellent delivery of care.
  • Coordinate the safe and effective delivery of outreach and satellite activities, developing and maintaining effective partnerships with other agencies.
  • Support the Deputy Service Manager and Service Manager with contributions to quarterly contract monitoring submissions, including case studies and data analysis.
  • Line manage the recruitment and welcome process for new joiners, conduct appraisals, and provide supervision to team members.

Requirements:

  • Proven experience in a similar role, with a strong understanding of case management and team leadership.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with colleagues and external partners.
  • Ability to work independently and as part of a team, with a strong focus on delivering high-quality services.
  • Knowledge of relevant legislation and policies, including safeguarding and data protection.
