About the Role

We are seeking a highly skilled and experienced Physiotherapist to join our team as a Community Rehabilitation Specialist. As a key member of our Therapy Services, you will play a vital role in delivering high-quality rehabilitation services to patients in their homes and communities.

Key Responsibilities
  • Provide safe, effective, and efficient interventions to patients with a range of conditions, including orthopaedic, neurological, and general long-term conditions.
  • Work closely with multidisciplinary teams to ensure seamless care coordination and patient-centered outcomes.
  • Develop and implement comprehensive rehabilitation plans, utilizing a range of treatment skills and options to promote independence and optimal health outcomes.
  • Provide education and support to patients, families, and carers to empower them to manage their conditions effectively.
  • Collaborate with other healthcare professionals to identify learning objectives and personal development opportunities.
  • Contribute to the development of team members and seek ways to improve services.
Requirements
  • Registered Physiotherapist with a recognized professional qualification.
  • Experience of working in community settings, including adult community rehabilitation.
  • Knowledge of relevant professional, national, and organizational guidelines and standards required for safe and effective service delivery.
  • Ability to manage own caseload effectively and delegate to rehabilitation assistants as needed.
  • Flexibility and ability to respond to changing service priorities.
