About the Role

We are seeking a highly skilled and experienced Occupational Therapist to join our team as a Band 6 Occupational Therapist in Adult Community Rehabilitation. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals with a range of conditions, including neurological and orthopedic injuries.

Key Responsibilities
  • Provide comprehensive assessments and develop individualized treatment plans to promote independence, health, and wellbeing.
  • Work collaboratively with patients, families, carers, and healthcare professionals to achieve optimal outcomes.
  • Manage a caseload of patients with complex needs, including those with frailty and falls prevention.
  • Develop and implement effective interventions to promote health and wellbeing, including equipment provision and minor adaptations.
  • Provide clinical supervision, education, and support to staff to identify learning objectives and personal development opportunities.
Requirements
  • Registered with the Health and Care Professions Council (HCPC).
  • Hold a recognized professional qualification in Occupational Therapy.
  • Experience of working in a multidisciplinary team and with other health, social care, and voluntary sector colleagues.
  • Knowledge of relevant professional, national, and organizational guidelines and standards required for safe and effective service delivery.
  • Ability to manage own caseload effectively and delegate to rehabilitation assistants appropriately.
