Job SummaryWe are seeking a highly motivated and experienced Senior Support Worker to join our team. As a Senior Coach, you will deliver person-centred support and lead a team of Support Workers to do the same. Your primary responsibility will be to manage a caseload of customers, supporting them to achieve their goals and aspirations.
Key Responsibilities- Manage referrals into the service, conduct interviews and assessments, and notify potential customers of the outcomes.
- Induct new customers into the service and deliver quality support sessions in one-to-one and group settings.
- Develop and implement individualised support plans to help customers achieve their goals.
- Work closely with the team to ensure seamless delivery of support services.
- Contribute to the development of the service, identifying areas for improvement and implementing changes as needed.
- Hold a Level 3 qualification in Health & Social Care (or equivalent).
- Proven experience of working with vulnerable young people with support needs.
- Excellent communication and leadership skills, with the ability to work in a professional manner with multiple agencies.
- Ability to work under pressure and on own initiative.
- IT literate and able to use Microsoft Office packages.
Appointment to this role will be subject to a satisfactory Disclosure and Barring Service Check, satisfactory references, and possession of a valid Right to Work document.
