Trainee Promoting Independence Worker

Working alongside Senior Promoting Independence Officers and other health and social care professionals along with external agencies to provide a personalised service to meet the patients individuals needs. Read, follow and implement recovery, re-ablement and rehabilitation plans clearly documenting the patients progress. To immediately escalate any changes to the appropriate health and social care professionals to identify if the patients require an immediate review. Depending on your competencies and skills you will be expected to work supervised or unsupervised in patients homes.
Encourage, inform and enable service users in relation to basic health, wellbeing and individual choices available to them. To provide information and signpost to other internal / external services that may meet individuals needs. Monitor and record service users' health and wellbeing and immediately escalate any concerns or changes in the patients condition, needs, or circumstances to the appropriate health and social care professionals
