Trainee Community Support Worker

The Home First service is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands NHS Partnership Foundation Trust (MPFT).
The service seeks to offer rehabilitation programme for a time limited period to rebuild functional range of movement, confidence, relearn new skills by promoting people's independence through health & social care interventions. The Home First Integrated service operates from 7am to 10pm over 365 days per year.
The post holder will be responsible for working in close collaboration with the Home First Multi-disciplinary Team which includes Nurses, Therapists, Care Support Workers and Social Workers.
The post holder will have exemplary communication skills, enabling them to be a role model for others, as well as positively representing the Trust values, in the delivery of effective, efficient and high quality services for service users. The post holder will be enthusiastic, passionate and a dynamic individual.
The shift patterns will be on a rotational basis over 7 days, so flexibility is required for this role.
- Promote a positive patient recovery, rehabilitation and re-ablement philosophy to ensure that every patient meets their potential level of independence.
- Working alongside trained promoting independent Officers to demonstrate your competencies and skills within recovery, rehabilitation and re-ablement within activities of daily living so that each individual patient meets their maximum potential level of independence.
- Working alongside health and social care professionals to ensure that all patients receive an individualised rehabilitation plan of care to meet their individual needs with realistic and appropriate goals
- Be able to identify and report any safeguarding vulnerable patient concerns in accordance to Policy.
- Have an understanding and contribute to outcome focused patient reviews
- Immediately escalate any patient concerns to the appropriate health and social care professionals.
- Demonstrate your competencies and skills to encourage individuals with the use of equipment & assistive technology

- Working alongside Senior Community Support Workers and other health and social care professionals along with external agencies to provide a personalised service to meet the patients’ individuals’ needs.
- Read, follow and implement recovery, re-ablement and rehabilitation plans clearly documenting the patients’ progress.
- To immediately escalate any changes to the appropriate health and social care professionals to identify if the patients require an immediate review.
- Depending on your competencies and skills you will be expected to work supervised or unsupervised in patients’ homes. As you build your competencies and skills within recovery, rehabilitation and re-ablement this will be reviewed and adjusted accordingly.
- Ensure that every patient’s safety and dignity is maintained at all times.
- Educate patients within managing long term conditions and work with the patient on their individual goals to meet their individualised needs and patients plan of care
- Encourage and provide information to patients in managing their own personal safety and document where positive risk taking identified and agreed with the patient.
- Encourage, inform and enable service users in relation to basic health, wellbeing and individual choices available to them. To provide information and signpost to other internal / external services that may meet individuals’ needs.
- Monitor and record service users' health and wellbeing and immediately escalate any concerns or changes in the patient’s condition, needs, or circumstances to the appropriate health and social care professionals
