**KEY DUTIES AND RESPONSIBILITIES**: Working alongside Senior Promoting Independence Officers and other health and social care professionals along with external agencies to provide a personalised service to meet the patients individuals needs. Read, follow and implement recovery, re-ablement and rehabilitation plans clearly documenting the patients progress. To immediately escalate any changes to the appropriate health and social care professionals to identify if the patients requires an immediate review. Depending on your competencies and skills you will be expected to work supervised or unsupervised in patients homes.

Encourage, inform and enable service users in relation to basic health, wellbeing and individual choices available to them. To provide information and sign post to other internal / external services that may meet individuals needs. Monitor and record service users health and wellbeing and immediately escalate any concerns or changes in the patients condition, needs, or circumstances to the appropriate health and social care professionals Comply with all policies, procedures and legislative frameworks The post holder is required to undertake training to ensure that they build the appropriate skills and competencies to provide high standards of recovery, re-ablement and rehabilitation To contribute to the achievement of the service Ensure a work environment that protects and promotes peoples health, safety and welfare To immediately report and escalate in accordance with Trust Policy any safeguarding concerns and promote the welfare of vulnerable adults. To personally be accountable for delivering services efficiently, and effectively Participation and contribution in your Personal Training and Development Conversations Ability to support with clinical tasks according to appropriate procedure and protocol following your ability to demonstrate appropriate competencies and skills within this role.

Ensuring high standards are maintained in a safe environment which promotes equality, equality and sensitivity for all individuals you will work with in this role All records must be kept in line with the health and social care records management policy.The post holder is responsible for keeping up to date and accurate health and social care records at the end of every care call.
