**SALARY**:Band 4 (£12.25ph)
**ACCOUNTABLE TO**: Network Managers
**REPORTS TO**: Network Managers
**JOB SUMMARY**
Reporting to the Network Managers, this role will be pivotal in supporting the multi
- disciplinary Network team in delivering the outcomes from the Network Direct Enhanced Service (DES) Agreements which comprise:
- Enhanced Health in Care Homes:
- Structured Medication Review and Medicines Optimisation
- Supporting Early Cancer Diagnosis
- Anticipatory Care
- Personalised Care
The role will have a strong focus on administrative co-ordination of these programmes of work within the network. The care coordination role supports the PCN leadership team and GPs in coordinating all key activity and will identify and organise a proactive approach to improve the care to our patients within the PCN.
Joining our existing team of Care Co-Ordinators, this particular role will take on the administration arm in supporting the services based at East Way including the Enhanced Access and continued growth of the Network.
**KEY RESPONSIBILITIES**
- The role will be based in the Network Hub at East Way Clinic and will work closely with the Network team comprising the Network Managers, Clinical Pharmacists, the Self-Management Team and the Assessment & Care Team (AC Team) as well as the Practice teams.
- The role will gain a thorough understanding of the requirements of the Network DES and support the Network Managers in developing a Work Programme to monitor progress against the outcomes and timescales.
- The role will require a good working knowledge of SystmOne in order to be able to set up and run SystmOne searches to identify the patient cohorts, as directed by the clinicians. You will also need to understand how to use the Dorset Intelligence & Insight Service (DiiS). Training will be provided if required.
- The role will also work in a person-centred way with patients and colleagues to encourage patient engagement in the quality improvement programmes as outlined in the DES and also contribute to a positive and seamless service-user experience, appropriately supporting those who may experience additional barriers to accessing the services.
- To support the Network and Practice teams with clinical audit and evaluation relating the Network DES clinical outcomes and quality improvement initiatives.
- To provide administration support to the Clinical Director and PCN Managers ie helping to prepare for meetings, research, ad-hoc searches and more.
- Take minutes at PCN-wide meetings as required.
- Support the Enhanced Access Hub located at East Way by ensuring clinical supplies are recorded and maintained.
- Monitor supplies for teams at East Way.
- Assisting the Network Managers with the recruitment of staff to the PCN.
- Co-Ordination of meetings between the PCN, practices and stakeholders.
- To undertake any other duties or projects commensurate with the nature and grade of this post as required.

- To be responsible for personal learning and development.
- To supporting digital initiatives and includes responsibilities for the co-ordination of the patient’s journey through primary care.
