The Hepatology team in NBT are delighted to announce this post for a band 6 Early Detection of liver cancer Pathway Navigator. It is fixed term position for 12 months with potential or extension. This role can e offered as a secondment or fixed term opportunity.

We are looking to recruit dynamic and enthusiastic full time Band 6 Early Detection of liver cancer Pathway Navigator will take responsibility for the implementation, delivery, and evaluation of a new service for cancer patients who are referred to the hepatology department at North Bristol Trust, They will be part of the wider hepatology team and will facilitate the smooth running of the patient journey by ensuring that coordinated and streamlined clinical and administrative processes revolve around the patient and their individual needs.

The post holder will develop a nurse led clinic to help manage the delay in early cancer detection and will be responsible for maintenance of surveillance databases for patients on the pre cancer pathway and those requiring HCC surveillance, ensuring timeliness of investigations and monitoring of progress along the patient journey. The post holder may also be expected to complete simple diagnostic investigations such as fibroscans blood tests and ultrasound requesting.
- Manage pathways for diagnostic investigations and appointments ensuring they are booked promptly and expedited as needed.
- Facilitate a reduction in unnecessary or ill-timed follow-ups and contribute to the reduction of the gastro outpatient backlog through proactive management of surveillance patients.
- Act as a single point of contact for hepatology patients and family members with regards to their treatment pathway developing specialist non-clinical knowledge to signpost patients appropriately for support outside of hospital.
- Work collaboratively with the clinical teams to provide a seamless service which is responsive to patients, carers, and relative’s needs.
- Manage databases of our chronic liver disease patients
- Run a supported clinic HCC Surveillance - In doing this we hope to avoid late presentations of HCC through proactive monitoring of cirrhotic liver patients, and identify HCC in a timely way that mean patients can access curable treatments
- Alleviate pressure on Consultant and Clinical Nurse Specialist time, enabling them to focus on delivery of clinical care.

**The post holder will**:
Feedback to NHS E and the SWAG cancer alliance at regular intervals through the year role.
- Support clinical teams and help identify and implement pathway changes that will improve compliance with national waiting times standards for cancer
- Track & chase diagnostic investigations and appointments for all patients on HCC surveillance pathway within the appropriate timeframes and regularly review and analyse waiting times across HCC screening pathways, escalating issues where necessary.
- Be knowledgeable on Hepatology conditions appropriate to the role such as Liver Cirrhosis, liver cancer, NASH, Hepatitis and be able to appropriately signpost patients in conjunction with the specialist nurse team.
- Provide a point of contact for patients through their surveillance
- Be responsible for organising appointments for diagnostic tests/treatment and communicate dates to the patient, providing relevant information prior to attendance to encourage attendance - and arrange follow up appointments with the relevant specialty for an appropriate time so that the patient can receive the results of any tests. CT scans, USSC, Fibroscans, blood tests, MRI, questionnaires, endoscopy, and biopsies
- Develop excellent working relationships with departments which deliver diagnostic services including radiology, endoscopy, and pathology to co-ordinate the patient’s diagnostic journey.
- Support patients and carers to access appropriate information and support, including sign posting to a range of support services. Inform patients and carers who and how to when they feel that their condition or needs have changed, as appropriate including what to do out of hours.
- Provide support and co-ordination for the HCC MDTs for efficiency. This includes recording of information from MDT in the clinical database and supporting recommended actions.
