Consult with patients and family members to discuss their health problems this may include checking in with patients and evaluating and documenting their progress Educate patients about their condition, medication and give them specific instructions Develop a care plan to address their personal health care needs, including vulnerable patients and those with mental health conditions Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care. This may include, where clinically safe and appropriate, using decision aids in preparation for any shared decision making conversations To co-ordinate recall lists for patients who are being monitored for chronic health conditions. This is a vital role in ensuring that patients do not fall through the net in long-term follow up To follow up patients who have been discharged from hospital to ensure all discharge actions have been carried out and to identify any additional needs to reduce the risk of readmission Support proactive case finding using population health intelligence data Work closely with professionals from a variety of health specialties to set up patient appointments and treatment plans Attend ongoing training and educational courses to keep abreast of new developments in health care Treat patients with empathy and respect and conduct oneself in a professional manner at all times Comply with organisation guidelines and health care guidance and regulation
