GDOC is looking for an Advanced Clinical Practitioner to join our GP surgeries based in Gloucester. The ACPs will be joining an ever expanding & developing team, striving to provide the best care for the people of Gloucester.

As an ACP Nurse, you'll be delivering top-tier patient care and adeptly managing acute and chronic conditions.

You will work autonomously across all 4 pillars of advanced practice, supported and supervised accordingly. The job plan will include no more than 80% clinical work, with the remaining 20% covering the other AP pillars, providing opportunities for ongoing professional development.

If you are joining up from outside of a Primary Care role we will support you in an appropriate induction period, to develop the required skills needed. We would also welcome any external skill sets as a forward thinking provider of health and care.

Your responsibilities as an Advanced Nurse Practitioner will include:

- Administering comprehensive, evidence-based care to patients of all ages, adhering to clinical protocols and guidelines.
- Undertaking initial assessments, diagnosing and overseeing acute and chronic illnesses, and prescribing tailored treatment plans.
- Performing thorough physical examinations, interpreting diagnostic tests, and collaborating with the healthcare team to devise comprehensive care strategies.
- Independently handling and triaging minor illnesses, and delivering prompt and efficient treatments.
- Request and interpret appropriate investigations to support clinical care, within your scope of practice.
- Make onward referrals for patients as required within a timely manner and action resulting hospital or specialist information and letters.
- Offering health promotion and preventive care services, encompassing vaccinations, health screenings, and personalised lifestyle guidance.
- Educating and supporting patients and their families, ensuring a clear understanding of conditions, treatments, and self-care measures.
- Collaborating closely with healthcare professionals, physicians, nurses, and specialists to streamline patient care and optimise outcomes.
- Maintaining meticulous and up-to-date medical records, in compliance with regulatory standards.
- Work autonomously making highly complex clinical decisions with mínimal supervision from GPs and the Chief Nursing Officer, working within local and national policies and being responsible for how these policies are interpreted.
- Actively engaging in clinical audits, quality improvement initiatives, research and ongoing professional development to elevate your clinical prowess, and maintain your skills with the advanced practice research pillar.
- Provide education and guidance to the nursing, medical and other members of the multi-disciplinary team as appropriate.
- Opportunities for leadership development and it is expected you would provide leadership within the clinical teams as appropriate.

**Qualifications**
- Essential:_

NMC or HCPC registration

Educated to Masters level or equivalent

University qualification in autonomous practice/clinical skills (eg University Advanced Clinical Practice Masters/Advanced Clinical Skills PGCert)

Hold or are eligible for an ‘Advanced’ digital badge issued by the Centre for Advancing Practice

Non-medical prescribing qualification

**Experience**

Significant clinical NHS experience,

Clinical leadership experience

Audit or research experience

Teaching and education experience
