**Job Purpose & Service Information**:
The vision of this service is to achieve a holistic, innovative, and flexible service to support Carers across Carmarthenshire to improve and maintain their health, well-being, and independence, through social integration.

The role offers an exciting opportunity to be involved in the development and delivery of Adferiad’s Carers Services across Carmarthenshire, through co-production and partnership working. The post-holder will work flexibly to engage with carers to ensure their voice is at the heart of the development.

The post-holder will work in a holistic, strength based, recovery focused way, fully engaging with the carer to identify what their needs are and what matters to them.

Support provided will focus on promoting independence, building resilience, and promoting community engagement. The post-holder will help individuals recognise their achievements, skills and strengths, and use these to help them identify and work towards future goals.

**Main Duties and Responsibilities**:
**- **Support carers using a holistic strength-based approach, to identify what matters to them and support them in developing their own wellbeing plan.
- Identify with the carer, goals outside their caring role enabling them to plan for their future.
- Support individuals to understand their caring role and to identify themselves as Carers, enabling them to access appropriate support.
- Facilitate carers support groups, focus groups and co-production exercises.
- To ensure carers are aware of their rights and responsibilities, including Carers Assessment and Carers Allowance.
- Ensure carers are enabled to exercise informed choices about their lives where possible and to enhance their social skills and encourage participation in the community.
- To work with carers to provide or arrange the support mechanisms needed for involvement in the planning, design, delivery, monitoring and evaluation of services.
- To assist carers in meetings, raising, discussing, and responding to issues about services and to participate in any other events where their involvement is required.
- To monitor, review and evaluate carers input and involvement in the above, using quantitative and qualitative methods.
- Campaign on carers issues and raise awareness about carers with Statutory and Third Sector organisations as appropriate.
- Provide information and advice to Carers on relevant health conditions and legislation relating to their specific needs.
- Liaise closely with statutory services and other third sector organisations to develop a support network for Carers.
- Provide access to training opportunities so the carer feels more equipped to support the cared for and feel more confident in their communication with professionals.
- Support the Carer in being involved in the planning and delivery of care and ensure they can voice their concerns about the wellbeing of the cared for to the team.
- Ensure that Carers and service providers are aware of the rights of carers particularly in relation to the issue of confidentiality.
- Identify with the carer other avenues of support that would benefit their wellbeing and signpost to appropriate organisations/services in a timely manner.
- Maintaining records and statistics required to monitor and evaluate the effectiveness of the service.

This is a part-time position. The average work week is Monday through Friday between 9am - 5pmm with no weekend work required.
