Senior Jet Practitioner

We are seeking dynamic senior RN’s or paramedics with amazing interpersonal skills to join our 2 - 4 hour admission avoidance service. We are looking to recruit new team members who are proactive and effective, working autonomously within the team.
The Joint Emergency Team (JET) work across the county, our aim is to prevent unnecessary hospital admission by providing high quality holistic assessments and diagnosis of conditions, managing patients with sub-acute conditions to aid their recovery from illness or injury, within their own home. We work closely with the specialist teams in the community, with GP’s and with the acute trust.
We are looking for experienced clinicians with a passion for a high quality of care and experience in either acute medicine, emergency medicine or community experience. The post holder will provide holistic assessments and interventions within the patient’s own home and will ensure the patients are referred onward as appropriate to community services once their care episode is ended.
You will work alongside colleagues with real expertise in this area of the community care. The team has a passion for improvement, innovation, and delivery of a high quality of care to patients in their own home and working within the wider multi-disciplinary team within the local community. We are looking for clinicians who are team players, have highly developed communication skills, are able to work flexibly and can prioritize their workload.

- To clinically assess, provide a working diagnosis, management plan in collaboration with GP's, emergency services and the Acute Trusts.
- Providing mentorship, management and supervision to junior colleagues, as well as clinical advice and support.
- Provide clinical triage for the service as required.
- Have an active role in the development of the service
- Work with service/operational leads to maintain governance /CQC requirements and bench mark service with other organisations.
- To be actively involved in the implementation of audits within the service
- Be innovative, forward thinking and proactive in the improvement and development of the service.
