Nelson Medical Group is a busy practice of just over 5000 patients and are looking to recruit Customer Care Coordinators within our team.

We require experienced primary care administrative staff to support our Practice and ensure data is coded and filed correctly. You will be based at Seaton Delaval along with a Team of Coders and working remotely for Nelson Medical Group

Our team currently comprises of a mixture of clinical and non-clinical staff who all work towards the same goals putting patient care at the forefront of everything we do.

The post-holder will assist in the implementation of patient care, analysing information typically in the form of discharge summaries and correspondence from clinicians/departments both within and outside of our organisation to ensure the data is coded and filed correctly.

To provide clinical coding of all patient records ensuring all medical history is coded accurately and entered onto the clinical system some of which is sensitive.

Nelson Medical Group is a busy practice at the centre of North Shields which has an increased elderly and unwell patient population.

We are one of nine practices who are part of Northumbria Primary Care or NPC.

As NPC is a wholly owned subsidiary of Northumbria Healthcare NHS Foundation Trust all eligible employees can access NHS Pension Scheme, Fleet Solutions lease vehicles, Childcare vouchers etc. In addition should you be interested in portfolio working within specialist areas of secondary care this is something we can support.

At Nelson Medical Group there is a team of four GPs and an Advanced Nurse Practitioner. The practice has a team of three nurses/Health Care Assistant who provide nurse led clinics for patients with long-term conditions. The clinicians are supported at the practice by a practice manager and a team of reception/administration staff. The practice is open between 8am to 6.0The0pm Monday to Friday.
- Provide and receive routine information where there may be barriers to understanding, for example, where a patients first language isn’t English or they may have an impairment.
- Acts professionally and appropriately in situations, understanding that there may be boundaries to confidentiality, seeking support and advice.
- Works with colleagues in other services to facilitate safe and effective transition of care between services.
- Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.
- Clinical coding of patient records, ensuring all medical history is coded accurately
- Able to document information in a legible, accurate and professional manner
- Develops partnership working in order to support the patient, carers and staff.
- Demonstrates knowledge of care pathways in order to enable effective response to queries.
- Carries out clerical duties required by the role
- Develops the ability to work across professional boundaries
- Support people to develop and implement personalised care and support plans
- Review and update personalised care and support plans at regular intervals
- Must be able to demonstrate the English language proficiency level required for this post
