**FTC**
**Homeless Prevention**
**Coordinator** who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.
No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

**_Fixed Term Contract until 31st March 2025_**
**Salary**: £26,393.77 - £30,316.74 dependent on experience (+ Outer London Weighting £2,080.07)
**Responsibilities**:
**About the role**:
- The post holder will provide an accommodation assessment for new or existing service users
who are at risk of being made homeless.
- The aim of the accommodation assessment is to identify homelessness or risk issues within
their accommodation which create a risk of losing their accommodation.
- The aim of the assessment and subsequent actions are to ensure accommodation is
maintained or, if it is not possible to maintain current accommodation or if the person is
homeless; to work with local housing agencies and staff to secure alternative accommodation
for those that are either homeless or at risk of homelessness.
- The post holder will develop and coordinate positive working relationships with statutory
teams, relevant partners, agencies including local authorities and homeless departments to
ensure accommodation and appropriate support outcomes are obtained.
- The post holder will adopt a case management approach and will work to achieve positive
accommodation and support outcomes and provide secure accommodation or ensure this
process is underway.
- Supporting service users to safely exit structured treatment and continue to work through
their aftercare plans to achieve their desired goals
- Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination
**About you**:
- Have a good working knowledge on Housing Legislation.
- Have a strong understanding of substance misuse issues and experience of working within a similar field
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills
- Have a good working knowledge of mental health interventions, services, and good practice
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Ability to manage change successfully in a way that prioritises the needs of service users
- Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
